• This document is used to capture the particulars of all your dependants in order for them to reflect on your policy. It is therefore crucial for the document to be clear and legible, in order to capture all information correctly.
• This also provides us with information regarding your dependant’s current health status and if they have had any prior and/or need any further procedures performed.
• Once all the information is captured and the policy is pending for the activation date for the dependant, you will receive a system-generated policy schedule reflecting the additional dependant’s details added to your policy schedule.
• If your dependant is not loaded as a dependant on your policy at the time of incident they were in hospital, they will not be covered for the procedure or treatment provided.
